Smiles Northwest Patient Information

For patient information at Smiles Northwest, we are always looking for ways to streamline patient processes, working to save you time and effort.

With this in mind, we now provide our dental patients with the opportunity to complete new patient forms prior to your scheduled appointment. This allows you to complete the forms in the comfort of your own home, without feeling rushed. It also means that you do not need to arrive for your scheduled appointment early (although we do ask that you arrive on time as a courtesy to other patients and to our hard-working dental team).

If you choose to fill out your forms at home, we can either email or mail them to you. You can then bring them to Smiles Northwest and give the forms to a team member when you arrive for your initial visit. Always feel comfortable calling if you have any questions or need assistance with the forms.

Insurance

As a courtesy to our patients, we accept most dental insurance plans and will gladly file your insurance claims for you. In order to process your insurance claims properly and in a timely manner, we must have your correct and appropriate insurance information.

We ask that you please inform us of any changes regarding your dental insurance when here at the Smiles Northwest office to receive dental care. Likewise, if you have a new insurance card, please make sure you let us know so we can update your file.

Please keep in mind, most dental insurances do not provide benefit for many cosmetic procedures. Striving to make all of our expert dental services available to everyone, we do offer payment options. Here at Smiles Northwest, we are dedicated to making sure our dental services are as affordable as possible, and accessible by all. We will work hand-in-hand with you to make sure you are fully informed about your treatment plan, or any procedure you may be interested in, and about all related costs.

This is undoubtedly a very exciting time for you. After all, changing your smile will change your life. While such results are truly priceless, we encourage responsible financial planning. Whether you have been saving to pay for your treatment, planning on using a low interest credit card to finance your treatment plan, or perhaps just need some direction in choosing the finance company that will fit your situation, we are here to help.

For your convenience, we gladly accept these forms of payment:

• Cash or Check (5% discount on services if used day of treatment)
• Credit Card (VISA, Master Card, DISCOVER and American Express)
